Class of:    (list all 4 digits)

Phone Number:  (a new feature to the database - don't forget the area code; this phone # item is optional)

Email Address:  (include up to 2 addresses that you would like published)

Mailing Address: (street, city, state, zipcode) & (if outside the USA, include country) 
 *******************  (if you have dual residence, you may enter a 2nd address) *********

If you do NOT desire to publish an email address, you must indicate a mailing address; or vice versa;
be sure to include your full name and class.

Information update or correction:

In order to submit a change be sure to include your full name and class.

Of course, provide the updated or corrected information.
